NEW PATIENT FORMS:
Please fill out the following forms and bring them with you to your first visit. If you would like to fill out the forms in our office before your visit, please arrive at least 15 minutes early to leave time to complete them.
Our office is located at 1020 19th St. NW, Suite 775, Washington DC 20036. (On 19th Street between K and L)
There is 2 hour street parking set up for use with the Park mobile app (which ends promptly at 4pm in most locations) and garage parking. Each garage charges different rates and there are several garages immediately across the street from our building. On your first visit, we recommend leaving extra time for parking.
Our practice is an out of network provider with insurances, meaning that the cost of the session is due at the time of service. As a courtesy, we have a professional biller who is able to submit claims to your insurance on your behalf.
OUR INSURANCE POLICY
Please note that we are not Medicare providers. If you have Medicare as your primary or secondary insurance and would like to be seen at our practice, you may come for a wellness visit. If you choose to do this, the cost of the visit will be out of pocket and you will not be able to file for reimbursement with either Medicare or an alternate insurance.
MEDICARE STATUS EXPLANATION
Appointments can be made either during phone hours by calling 202-803-2068, by emailing requests to email@example.com Patients are responsible for knowing their scheduled appointments. Reminder emails will be sent as a courtesy but we are not responsible for any emails that are not received or end up in bulk/spam email.
BOOK AN APPOINTMENT
In the event that you are unable to attend your appointment, we require at least 24 hours / one business day advance notice. When you schedule an appointment with our practice, that time is reserved for you. When you miss the appointment without calling to cancel within a reasonable amount of time, your practitioner does not have the opportunity to offer that time to someone else in need of services. Missed appointments can also interfere with your progress in treatment. It is our policy that patients are responsible for all appointments that they have scheduled. IF YOU FAIL TO GIVE US 24 HOURS/ ONE BUSINESS DAY ADVANCED NOTICE, YOU WILL BE CHARGED FOR THE COST OF THE VISIT. Fees for missed appointments and/or late cancellations are expected at or before your next scheduled appointment. Insurance does not cover these fees. Any exceptional circumstances will be submitted for review.
Patient will be charged the fee for the entire scheduled appointment regardless of the time that the patient arrives. If we can we will do our best to accommodate your time but this is not a guarantee.
