NEW PATIENT FORMS:

Please print and fill out the forms which pertain to you and bring them with you to your first visit. If you would like to fill out the forms in our office before your visit, please arrive at least 15 minutes early to complete them.

Our office is located at 1020 19th St. NW, Suite 775, Washington DC 20036. (We are on the west side of 19th Street between K and L Streets.)

Metro:  Red line (Farragut North Station), Orange/Blue/Silver Line (Farragut West Station)

PARKING
Metered street parking in 2-hour increments is available, but can be difficult to find. Many metered spots are only available from 10am -4pm. Meters accept credit cards and the ParkMobile app. There are several garages across the street and on the same block as our building. We recommend allowing extra time for parking on your first visit.

 

INSURANCE
Our practice is an out of network provider with health insurance companies, meaning that the cost of the session is due at the time of service. As a courtesy, we have a professional biller who is able to submit claims to your insurance on your behalf.

OUR INSURANCE POLICY


Please note that we are not Medicare providers.  If you have Medicare as your primary or secondary insurance and would like to be seen at our practice, you may come for a wellness visit.  If you choose to do this, the cost of the visit will be out of pocket and you will not be able to file for reimbursement with either Medicare or an alternate insurance.

Appointments can be made by calling 202-803-2068 during business hours or by emailing requests to info@thriveagainpt.com Patients are responsible for knowing their scheduled appointments. Reminder emails will be sent as a courtesy but we are not responsible for any emails that are not received or end up in bulk/spam email.

APPOINTMENT CANCELLATIONS
In the event that you are unable to attend your appointment, we require notice at least 24 hours or one business day prior to your appointment.

When you schedule an appointment with our practice, that time is reserved for you. When you miss the appointment without calling to cancel within a reasonable amount of time, your practitioner does not have the opportunity to offer that time to someone else in need of services. Missed appointments can also interfere with your progress in treatment. It is our policy that patients are responsible for all appointments that they have scheduled. If you fail to give us 24 hours/ one business day advanced notice, you will be charged for the cost of the visit. Fees for missed appointments and/or late cancellations are expected at or before your next scheduled appointment. Insurance does not cover these fees. Any exceptional circumstances will be submitted for review.

LATE ARRIVALS
If you are late to a scheduled appointment, we will still be able to meet with you, but our appointment will end at the scheduled time. You will be charged the fee for the entire scheduled appointment regardless of the time that the you arrive.
